How the MLTSS process works for Elizabeth residents
Elizabeth residents who are not already on NJ FamilyCare should begin through Union County aging/disability or social-services resources, then confirm whether a prospective assisted-living residence participates in the relevant Medicaid arrangement.
Already have NJ FamilyCare?
New Jersey directs current Medicaid members to contact their assigned managed-care organization to initiate an assessment for MLTSS eligibility.
Do not currently have Medicaid?
For adults age 21 and older, New Jersey directs applicants to their County Aging and Disability Resource Center or County Social Service Agency to start the MLTSS eligibility process.
Then verify the provider
Eligibility does not automatically establish that a specific assisted-living residence participates in the member's arrangement. Check the state database, the MCO and the provider.
What clinical eligibility means
New Jersey states that adults age 21 and older must meet a nursing-facility level-of-care standard for MLTSS clinical eligibility. The state's current description includes requiring hands-on assistance with three or more activities of daily living, or having cognitive deficits that require supervision and cueing with three or more activities of daily living.
This page does not determine whether a particular person meets that standard. Eligibility is established through the official assessment process.
Financial eligibility is separate
Financial requirements include income and liquid-asset rules. Those rules can change, and personal circumstances matter. Use current NJ FamilyCare guidance rather than a static city webpage for a final eligibility determination.
How to verify assisted-living participation in Elizabeth
| Step | Action |
|---|---|
| 1 | Use New Jersey's official long-term-care facility search and choose the assisted-living facility type. |
| 2 | Search by city or county and use the Medicaid funding-source filter where appropriate. |
| 3 | Confirm the current funding/participation status directly with the residence. |
| 4 | Confirm with the member's MCO that the proposed service/provider arrangement can be coordinated. |
| 5 | Ask what charges remain the resident's responsibility. “Accepts Medicaid” is not the same as “every monthly charge is paid.” |
What Medicaid participation does not tell you
A Medicaid funding indicator does not tell you that the provider has an available room, that it can safely manage the resident's care needs, that a particular unit is covered, or that every housing and service charge will be paid. Those are separate questions.
Room, board and service-cost questions
Do not convert the phrase “MLTSS includes assisted living” into a promise that an entire private assisted-living bill disappears. Ask the MCO and provider exactly which services are covered, what the resident must pay, and how room-and-board charges are handled in the specific arrangement.
Elizabeth versus nearby Union County options
MLTSS eligibility rules are statewide, but provider availability is not. A family in Elizabeth may need to compare participating options elsewhere in Union County or a practical nearby area if the local choice does not meet the resident's care needs.
Why this page is not a doorway page
This page has a distinct local Medicaid/MLTSS purpose: it explains the county-level entry pathway, the statewide eligibility framework, how to verify local provider participation and the difference between program eligibility and provider availability. It is not a city-name version whose only job is to send visitors to the assessment.
Frequently asked questions
Does MLTSS include assisted living?
Yes. New Jersey's official MLTSS materials list assisted living among covered long-term services and supports.
Does every Elizabeth assisted-living residence accept Medicaid?
No. Participation must be checked for each provider and the member's actual managed-care arrangement.
Where do I start if I already have NJ FamilyCare?
Contact the assigned managed-care organization to initiate an MLTSS assessment.
Where do I start if I do not have Medicaid?
Adults age 21 and older should begin through the county Aging and Disability Resource Center or County Social Service Agency.
Can a facility approve my MLTSS eligibility?
No. A provider can discuss admission and participation, but official eligibility is determined through the state program process.
Does Medicaid funding guarantee an available bed?
No. Availability, admission criteria and care fit must be verified separately.

Start With Eligibility, Then Compare Providers
Separate the eligibility question from the provider question. Both have to work before a placement is realistic.
