Knowledge-operations rules
- Existing canonical page first. A new URL is a last resort, not a participation trophy.
- Phase 14 publish gate remains mandatory for every new indexable page.
- Time-sensitive provider, payment and cost facts carry explicit review dates.
- GSC measures Google Search demand. Direct AI observations measure AI citations. These remain separate datasets.
- Every substantive change should be logged so later ranking/citation movement has an audit trail.
- Repeated AI misses become Phase 17 gaps, not instant new pages.
